A man in his early forties used to be sharp. He worked out, slept fine, and felt driven. Now he's dragging through the day, can't focus, and for the first time in his life he's carrying weight in his belly. His sex drive isn't what it used to be. He goes to his doctor, they run labs, and the call comes back: your testosterone looks normal.

He hangs up thinking the problem must be him. But what if it's not him. What if the problem is what "normal" actually means on a lab report.

I hear a version of this every week at the pharmacy, and not just from men. A postmenopausal woman in her 50s has been to multiple providers. She's exhausted. She's lost her libido. She's losing muscle and gaining fat, and her mood feels flat. She's been told it's estrogen, maybe progesterone, maybe try an antidepressant. Nobody mentioned testosterone. That's a blind spot, because women make testosterone too, and in a lot of women it actually circulates at higher levels than estradiol. It's barely part of the conversation.

Why "Normal" Doesn't Mean Optimal

Patients see "normal" on a lab report and figure that settles it. But total testosterone is only one piece of a much bigger puzzle, and honestly it's not even the most important piece.

Most of the testosterone in your blood is bound to a carrier protein called sex hormone binding globulin, or SHBG. I like to think of SHBG as a hormone taxi. When testosterone is tightly bound to it, it's not free to actually get into your tissues and do anything. So you can have a total testosterone number that looks perfectly fine on paper while your free testosterone, the part your body can actually use, is low.

That matters for men. It matters even more for women, because a woman's testosterone can look completely normal on paper while a high SHBG quietly makes her free testosterone functionally low.

So if you've been told your testosterone is normal, don't stop there. Ask if they checked free testosterone, SHBG, estradiol, estrone, and DHT. Ask when your labs were drawn relative to your last dose. Total testosterone by itself just isn't enough to make a real decision from.

 

What You Actually Need to Know

It's Not Just a Sex Drive Hormone

People hear testosterone and think libido, muscle, aggression. That's the cultural version, not the biological one. In men it plays into erectile function, sperm production, bone density, glucose metabolism, mood, focus, and recovery. In women it affects libido, muscle maintenance, bone strength, tissue repair, and even confidence through its effects on the brain. Sex drive is usually the first thing people notice, but it's nowhere near the whole picture.

Where It Actually Goes Once You Take It

Testosterone doesn't just sit there and stay testosterone. An enzyme called aromatase can convert it into estradiol. Another one, 5-alpha reductase, converts it into DHT. This is where most people run into trouble. If a man starts testosterone and develops breast tenderness or fluid retention, I want to know what's happening with his estradiol. If he's getting oily skin, acne, or hair loss, I want to know his DHT. If a woman develops acne, facial hair, or changes in her voice, I want to know her dose, her route, her free testosterone, her SHBG, and her DHT. More testosterone isn't automatically better. A lot of the time it just means more conversion into something you didn't actually want more of.

Men Need Estrogen, Women Need Testosterone

A man with zero estrogen isn't optimized. He's hormonally broken in a different direction. Estradiol supports libido, erectile function, sperm production, bone health, and brain health in men. That's why I'm not a fan of jumping straight to an aromatase inhibitor like anastrozole every time a man's estradiol comes up on testosterone therapy. Sometimes it's the right call. A lot of the time it's just the lazy call. The better question is, why is estrogen high in the first place? Is the dose too high? Is visceral fat driving aromatase activity? Is alcohol a factor? Is there insulin resistance underneath it? Lowering the dose or splitting the injection schedule solves this a lot more often than people think, without adding another drug on top.

Dosing and Delivery Actually Matter

For men, compounded testosterone cream doses usually start around 20 to 25 milligrams and can go up to 100 to 120 milligrams once daily in the morning, though absorption varies a lot from product to product. For women, topical testosterone is usually dosed between half a milligram and 2 milligrams once daily, and the site matters. I recommend the inner thigh, the back of the knee, or the back of the calf, rotated, because applying it to the same spot every day can coarsen or darken the hair there. Injectable testosterone works well for men, but I'd rather see it split weekly or even twice weekly instead of one big dose, because that reduces the peaks and troughs that drive extra conversion to estrogen and DHT. I'm not a fan of oral testosterone tablets for women. First pass metabolism through the liver destroys close to 99.9 percent of the dose before it ever gets into your bloodstream. And I'm just not a big fan of pellets either. They load three to four months of hormones into your body at once, so you're often way too high the first month and falling off a cliff by month three, which isn't how your body was ever meant to run.

Fertility Is a Conversation Men Need to Have Before They Start

Exogenous testosterone can act like male birth control. It tells your brain to stop making LH and FSH, the hormones that signal your testes to produce testosterone and support sperm production. Sperm counts can drop significantly, and getting that back after stopping isn't guaranteed to be quick. If having kids matters to you, options like HCG or clomiphene can support your body's own testosterone production through the pituitary without shutting down sperm production the way direct replacement can. If your provider hasn't brought this up with you, bring it up yourself before you start.

 

The Practical Takeaway

If you're a man dealing with fatigue, brain fog, belly fat, and low libido, don't stop at a total testosterone number. Ask for free testosterone, SHBG, estradiol, estrone, and DHT, and get sleep apnea ruled out, because it can both cause and worsen low testosterone.

If you're a postmenopausal or perimenopausal woman dealing with low libido, fatigue, muscle loss, or a flat mood and nobody has ever mentioned testosterone to you, ask why. It's not a guaranteed fix for everything. But it's a real, legitimate part of the conversation your provider should already be having with you.
